Required materials (checklist)

1. The completed and signed 3-page application form.
2. An official academic record/transcript sent directly from the medical school to the Committee Chairman (first-year students may submit documentation that they will be attending medical school instead).
3. Any additional items listed below depending on whether you are a first-time applicant or a re-applicant.

First-time applicants must also submit:
- A brief paragraph describing the medical area(s) that interest you and your career goals.
- Names, phone numbers, and email addresses of two personal references (not family members). These references may be contacted about your character, motivation, and potential.

Re-applicants must also submit:
- A brief paragraph updating the Committee on your current medical interests and career goals.
- Re-applicants do not need to submit references again.

Notes and tips (from a teacher)
- Ask your medical school early to send the official transcript directly to the Committee Chairman — transcripts sent by the student will not meet the requirement.
- Put the application deadline on your calendar (fourth Friday of June) and aim to submit a few days earlier to avoid postal or technical delays.
